This increases the usage of PDPageContentStream for generating appearance
content, rather than manually writing out PDF tokens. The remaining
functionality related to default content strings has been moved into
DefaultAppearanceHandler and the class renamed to PDAppearanceString.
Note that AppearanceGeneratorHelper will no longer pass-through DA tokens
automatically, they will need to be set in
PDAppearanceString.writeTo(PDPageContentStream). Currently only fonts are
handled.
