Some images:
- gs-bugilla691763.pdf, gs-bugzilla689439-alphamask.pdf, gs-bugzilla693322.pdf: 
very dark (maybe related to what you mentioned "revert those values")
- PDFBOX-994.pdf is better
- PDFJS-5526-p11-13.pdf has a black bar on p3
- PDFBOX-2103.pdf is better but the drop shadows are very dark
- PDFBOX-2658.pdf face on top right now completely gone

Re "when to revert those values" - just an idea: could this be a RGB vs CMYK 
problem? In RGB "doing nothing" means black, and "do all" means white, but in 
CMYK it's the opposite, i.e. "do nothing" means white, and "do all" means black.
