I can confirm the effect, which happens only when printing, not when viewing.
My bet was on the type 3 fonts, these are made of PDF content streams. But I
tried with another file (from PDFBOX-2794) and printing was quick. However
there's one difference between your file and the one from PDFBOX-2794: in
yours, the type 3 charprocs have inline image masks, while the ones from
PDFBOX-2794 use vector graphics commands.
