I would like to second the proposal of Tilman to use the currently best working
solution (as verified by his tests) using BBox and CapHeight in the rare cases
where BBox has unusual large values. Since PDF was not designed for text
extraction it is normal to use some heuristics to cope with the shortcomings
produced by different PDF creators.
It might be good to later change to using the own calculated glyph bounds but
this has to be tested and optimized for performance as well as how this
influences the text extraction quality (algorithms might have to be adjusted as
well). So I don't see this can/should be done in the 2.0 RC cycle but will be a
target for 2.1.
