Attached patch file. This file fixes the field renaming when merging AcroForms, 
while also optimizing a bit on the nextFieldNum calculation and adding the 
ability in the PDFMergerUtility API to set the desired renaming prefix and to 
turn off renaming altogether. Turning off the renaming was added in support of 
PDFBOX-3111.

The patch also contains a simple unit test built on the above linked PDF files. 
These files are probably too large to be ideal, but they are what I had on hand.
