I'll have a look with trying with Arial later; I don't have it installed right 
now. LibreOffice is 5.0.6.2. The only reference to PDF/A on export I see is 
"Archive PDF/A-1a (ISO 19005-1)", but unfortunately it looks like that option 
is mutually exclusive with getting the PDF form created. There is another 
"Tagged PDF (add document structure)" option that is available also when 
creating the form, do you think that could be useful to test with?
