Please open another issue. You're not doing anything wrong. I suspect that the
resource dictionary is missing in your PDF. What would be useful is a
screenshot from the PDFDebugger command line application. Click View, Show
internal structure, then look for the field in "Root/AcroForm/Fields". In the
field, it can be found in "AP/N".
