Fields missing after running:
PDDocument doc = PDDocument.load(new File(input));
PDAcroForm acroform = doc.getDocumentCatalog().getAcroForm();
if (acroform != null) {
acroform.setNeedAppearances(false);
acroform.refreshAppearances();
}
doc.save(output);
doc.close();
