Problem submitted by "Duke DAI" on the user mailing list. He has code that
creates an image in a form field and then flattens. In 2.0.9, the image appears
after flattening; in 2.0.11, it doesn't.
(Note that the image never appears in Adobe Reader before flattening because
Adobe Reader considers it to be empty form field. PDFBox always displays the
image before flattening)
