creates an image in a form field and then flattens. In 2.0.9, the image appears 
after flattening; in 2.0.11, it doesn't.

(Note that the image never appears in Adobe Reader before flattening because 
Adobe Reader considers it to be an empty form field. PDFBox always displays the 
image before flattening)
