Here are the corrections I did:
- "1 rg" => replace with "1 g" or "1 1 1 rg" ("g" for one gray color, "rg" for
three rgb colors)
- page content stream not needed
- BBox should be 0 0 150 150, not what you did
- In the /AP, before "BT" add "0 rg" so that the text is black
