Here are the corrections I did:
- "1 rg" (probably meant to be white) => replace with "1 g" or "1 1 1 rg" ("g"
for one gray color, "rg" for three rgb colors)
- page content stream not needed
- BBox should be 0 0 150 150, not what you did
- In the appearance content stream, before "BT" insert "0 rg" so that the text
is black
