That sounds like you're doing something wrong when _trying to add the visual 
signature appearance in all the pages_.

Actually one can argue that already attempting to do so is the error; ISO 
32000-2 after all requires *signature fields shall never refer to more than one 
annotation.* (There is a way to weasel out of this but that still clearly goes 
against the intention of the authors of the specification.)

Most likely, though, there also is a bug in your code doing that. But without 
that code one can hardly analyze it.
