The bad image at the mid right is at
{{Root/Pages/Kids/[0]/Resources/XObject/Fm1/Resources/XObject/Fm2/Resources/XObject/Im8}}.
There is a rotation, so I suspect this is related to the unsolved problem of
the gs-bugzilla693033.pdf file of PDFBOX-3000. The usage is

I'll try to create a reduced PDF but I am pessimistic whether this can be
solved.
