On page 2, there is a checkmark at item 3 ("Ik verklaar dat ik dit formulier
naar waarheid heb ingevuld"). The value of /V and /AS are "Akkoord". After
running the AcroFormDefaultFixup and rendering the PDF, the checkmark is gone
and /V and /AS are "Off". (This value change does not show immediately in
PDFDebugger, but it can be seen when switching the views between "show pages"
and "show internal structure").
