[~tilman] yes sorry, the error occurs when saving the doc with .save() method
of PDDocument.
It goes all the way to the writeObject() method of COSWriterObjectStream and
ends up in the else statement L236-239:

And that's what's weird, when opening the file with an editor I can't find any
OutlineItems. 130 0 obj doesn't show up, neither 138 0 obj or the others.
I only find the Outlines reference 129 0 R
