i think we should leave USE_ITHREADS as-is. i would expect the majority of overhead to be in perl itself with USE_ITHREADS.
which all comes to:
if you want the best performance out of prefork mpm, build your perl w/o ithreads, which you don't want enabled as it slows perl itself down.
