All links etc are updated to use TLP apart from release download links or where
jar name is mentioned, these would be updated once a release is done as TLP.
Team page source is changed to md from xhtml which is easier to edit and is now
at:
http://svn.apache.org/viewvc/phoenix/site/source/src/site/markdown/team.md?view=markup
