Summary: Get rid of KeyValueBuilder interface

It's not used on the server side and the place where it's helpful on the
client-side (for multiple immutable indexes) would never be searched. We'd just
need to conditionally use this format for immutable indexes so that
SYSTEM.CATALOG wouldn't use it (as currently the KeyValues in the mutations are
searched sometimes).
