PHOENIX-4755 added two new classes, CustomAvaticaServerConfigurationTest and
AvaticaServerConfigurationFactory. Both are missing Apache licenses.
In addition to causing HadoopQA noise for all other commits, this is a blocker
for release because Apache doesn't have the rights to these classes without the
license header.
