If you want to leave an index active while rebuilding (e.g. an immutable
index), the IndexTool selects from the index table instead of the data table.
This is because it uses PostIndexDDLCompiler which builds an UpsertSelect
statement with a NO_INDEX hint, but the IndexTool only uses the Select portion
which has no hint.
