Description: Split the conflated {{SERVER FILTER BY FIRST KEY ONLY AND
<expr>}} line and rename the projection optimization lines to {{SERVER
PROJECTION FILTER BY FIRST KEY ONLY}} and {{{}... EMPTY COLUMN ONLY{}}}, each
followed when a value predicate exists by a separate {{SERVER FILTER BY
<expr>}} line for each predicate. {{ExplainPlanAttributes}} narrows
{{serverWhereFilter}} to the bare predicate and adds
{{serverFirstKeyOnlyProjection}} and {{serverEmptyColumnOnlyProjection}}
booleans, with matching {{ExplainPlanTestUtil}} fluent assertion getters,
