The problem in the sample file (attachment id=23710) was that the margin records are not found with the rest of the PageSettingsBlock. If Excel re-saves the file, it corrects that problem. POI now does the same. I'm not sure if if is of concern to you, but my Excel (2007) reports the data loss in the sample file ("Shared workbook revisions and custom workbook views may have been lost").
