Backport to 6.0.x of #1517, #1518, #1516 and #1519 (cherry-picked with `-x`). Conflict resolution: `TestVBAMacroReader` on trunk also holds `badModuleOffsetFromLaterDirStream` and `badDirStreamIsIOException` from #1497, which isn't backported. Those two tests are left out.
