The provider for the file:// protocol should be in ucpfile1.dll, which
is a UNO component shared library and must be registered during
packaging. To verify, search for com.sun.star.ucb.FileContentProvider
in services.rdb using "regview service.rdb / > services.rdb.txt" or
similar in the final installation.
You may also check whether you have a valid
<ooo>/share/registry/data/org/openoffice/ucb/Configuration.xcu.
