The exception handling in the disconnection process may lead to failure to
remove all NotificationListeners. Additionally, the disconnection process does
not not actually close the JMXConnector. A combination of these issues could
lead to multiple subscribed NotificationListeners to the same server across
multiple JMXConnectors.
