example qmf-agent occasionally crashes on shutdown

The example qmf agent (cpp/examples/qmf-agent) incorrectly does all of its
cleanup processing during a signal handler. This is not thread safe. Will
result in occasional crash on shutdown.
Will post patch shortly.
