I'd like to merge the following fixes from trunk to the 0.22 release
branch. They are all entirely confined to the 1.0 codepaths and are
relatively well contained within that and of low risk[1].
QPID-4674: Detect asynchronous connection close, session end and link

QPID-4675: Allow receivers from direct exchanges without a subject
filter, http://svn.apache.org/r1462139
QPID-4669: have cmake build require proton 0.3 or greater,
http://svn.apache.org/r1462214
QPID-4679: Cleaned up processing of addresses, including errors and
warnings where elements can not be supported,

[1] The last one is the largest delta in terms of lines changed, but
logically it only deals with the configuration of terminus and a lot of
the patch is just neatening up the code to remove duplication.
