Summary: Make operational logging consistent with new model,

The operational logging system needs an overhaul to reflect the more mature
object model underlying the broker.
It needs to distinguish properly between:
* object creation and subsequent recoveries.
* report state changes in a consistent manner.
The implementation should aim to be centralised and merely listen to the model
and react accordingly. Existing implementation objects should be relieved for
their operational logging responsibilities, removing the need for our current
exhaustive operational logging system tests.
