connections to virtual hosts which are still activating

Currently when a client attempts to connect to a virtual host which is still
activating then the connection is immediately closed. Instead the broker
should distinguish between hosts which are in the process of activating, and
those that are in a more long term non-active state. Where the virtualhost is
still activating, the broker should not immediately respond, but instead defer
any further action until the activation has completed.
