I can now answer my own questions - having read the AMQP spec a bit more. The 
trace that Ganesh is seeing is indeed what you would expect:
 * In most of the dispositions the receiver is settling messages - this means 
that it has forgotten them, therefore there is no reason to send a disposition 
frm the sender to receiver to say the sender settled them - in fact it is 
pointless as the receiver has already told us it has forgotten them and so 
couldn't do anything with the settled dispositions anyway.
 * In the case where the receiver disposition gets split into a release 
disposition and then a release and settle disposition, it is expected that the 
sender should respond to the release only disposition with a settle disposition 
as the release disposition is a terminal state and so the sender can now forget 
the delivery.

However this second point is true whatever the behaviour of the delivery 
auto_settle flag. So we should be seeing that disposition always in the split 
case. The only possibility where the sender disposition wouldn't get sent is if 
the sender processes both receiver dispositiions "in the same batch" and so 
knows that the receiver has settled before it would send its own disposition 
frame - I'm not sure if this is likely or not.
