Summary: log when/where a failover connection is restored

When a connection is initially connected, we log out the connection id and
server connected to at info level.
When a failover URL is in use and the client is reconnecting, we log out some
details of what is going on (various details at different log levels). We do
not however log an explicit matching info message that the connection was
restored to whichever it was (though some of the tests do achieve this through
use of internal listener mechanism).
We should add a simple log message upon reconnect aligning with the initial
connection message.
