SA/SU lifecycle events are never fired

Servicemix core supports life cycle listeners for both service assemblies and
service units (ServiceAssemblyListener and ServiceUnitListener). However,
majority of the events defined therein are never fired by the servicemix core.
The following were never fired in my settings and I have not found any code in
the sources that would seem to fire them: SA deployment, SA undeployment, SU
deployment, SU undeployment, SU startup, SU stop, SU shutdown. On the other
hand, SA startup, SA stop and SA shutdown events are fired correctly (as can be
found in the sources of the ServiceAssemblyLifeCycle class).
