Description: When the PubQueueCache is closed, the underlying head poller
(RangePoller) is not closed. This issue will only manifest when the cache is
closed while an historical page is being fetched. This scenario is more likely
when requests to the journal are being throttled. (was: When the PubQueueCache

been completely fetched. This situation can happen when throttling can happen
at the journal level. )
