The SitemapServlet has issues adding on-demand sitemaps of nested sitemap roots
to the sitemap index.
1) If a top level sitemap root contains nested sitemap roots and all sitemaps
including the one(s) of the top level sitemap root are served on demand, the
top level sitemap root is not added to the index
2) If the top level sitemap root contains nested sitemap the nested sitemaps
are added with their full resource path, not the one of the top level sitemap
root
