Triage: Not reproducible on demand, but apparently does happen enough to
occasionally get a debug on it. Unlikely it can be fixed for 3.1.0, and happens
rarely enough that it's not a critical problem. Setting target milestone to
3.1.1. Note that bug 3966 seems to report the same problem (closing that one as
dupe), and bug 4445 looks very similar and may be related.
