I've just realised that the FORGED_MUA_OUTLOOK and MISSING_MID rules only
started being triggered since my sys admin replaced psa-spamassassin with a
different spam assassin front end ('Atomic Scanner') a few days ago.
It didn't seem to change any .cf files on our existing spam assassin
installation, however it does go via qmail-scanner.
I'm not sure of the technicalities, but I'm guessing qmail-scanner and/or
Atomic Scanner may be somehow causing this problem.
Due to other issues we've had with Atomic Scanner in the last few days we're
removing it (in about 12-24 hours from now) and we'll revert to using spam
psa-spamassassin.
I'm not sure if you want to close this off or keep it open to figure out if it
really is a bug, but I'm happy to provide more details if you need them.
