Ok, looking at this bug report, I do see one thing missing..

Where's the whitelist_from_rcvd statement that the message was supposed to be
matching, but didn't?

I'm assuming you're trying to match the fidelity bit. That will *never* happen.

whitelist_from_rcvd requires that the MTA that receives a message do a
reverse-dns lookup on the host.

Looking at this header:

Received: from cs-us102.fidelity.com ([192.223.226.85])
        by mx-roseate.atl.sa.earthlink.net (EarthLink SMTP Server) with ESMTP
id 1k7Fl41nE3Nl34f1

Earthlink isn't bothering, or the header would look like:

id 1k7Fl41nE3Nl34f1

Sorry, SA can't do much to make up for lack of effort on the part of Earthlink.
