Looks like it is coming from an Exim interface to SpamAssassin (possibly through cPanel). The _REPORT_ macro (or the $status->get_report) produces a multi-line report, possibly including empty lines. Such report was not supposed to go into a mail header section, but into a mail body. If some interfacing code reformats it into a form suitable for inclusion into a mail header section, it must do so correctly, conforming to RFC 5322 rules. According to your example it seems the empty line in a report was not properly handled, and pushes the rest of the report into a mail body. Btw, the "Content preview:" is not a sub-header.
