Stratos should remove instance in ERROR state before trying to re-launch instance

Hi Dev,

I just filed STRATOS-1293<https://issues.apache.org/jira/browse/STRATOS-1293> 
for $subject.   This issue can exhaust resources on our Openstack cluster.   
Not sure if it’s been fixed on the 4.1 branch.
