That looks good, perhaps http://people.redhat.com/caolanm/marginnotes/ would be helpful to you to see a very hacky non-functional stab at this I made recently which might help point out some of the relevant objects and classes involved in notes. Positioning of the notes is an interesting problem, how to lay them out when there's more than one on a line and if there's isn't enough space on the page to show their full contents.
