Expose number of active connections per endpoint

it would be useful to get list of endpoint synapse is connected to. I would
like Synapse to expose this metric through JMX.So that at any given point we
can tell which backend pool has most number of active connections.
In most cases this will also tell that the backend pool is having issues and
should be SwitchedOff in Synapse.
