Summary: Simplify samples with respond mediator

Before the respond mediator, it is required to add 3 mediators (or 4) to send a
response to the client in the request path. With the respond mediator, that
requirements is not there anymore. Hence, it is possible to simplify the
samples with 3, 4 mediators. Following samples were identified.
