The tutorial states that the default page of Tapestry application is named
"index".
Actually, this is not the case.
Instead, "Start" is the default page, and the tutorial1 artifact generates
Start.tml and Start.java.
I believe every mention on "index" should be replaced with "start" in the
tutorial1 documentation.
