As [~tilman] noted on the dev list, some of the reports are confusing because
they only contain the "container" file path, when the information may be about
an embedded file. We should eventually include the embedded file id and the
embedded file path...we can do that on another ticket.
For this ticket, let's add the existing field "file_name" to those reports that
include file by file information.
