Review Comment:
Deliberately not clamped. The endpoint lays its defaults *under* the request
configuration, which is the point of the design: a caller tunes dpi or width
for the thumbnail it wants, and the defaults only fill what the caller left
open. Overriding that here would make the endpoint the one place where a
configured server ignores its client.
The amplification also needs the operator to have turned per-request
configuration on in the first place, and once it is on, the same request works
on /unpack and /rmeta, where far more is rendered and returned. What this
endpoint adds on top is a cap on what it hands back: 32 MiB, or 413.
