I have been able to compare this test case with the current Tomcat test case that should test this scenario and have spotted one difference - the error handler throws an exception in this case. I have updated the Tomcat unit tests to include this additional case. The no so great news is that this has uncovered some further edge cases where onComplete() isn't being called and fixing all of these is looking non-trivial.
