TS does not process must/proxy revalidate correctly

Currently, even though TS stores items with must/proxy revalidate in the cache,
they are never served out, when in fact the items can be stored. Correct
handling as per http://www.w3.org/Protocols/rfc2616/rfc2616-sec14.html#sec14.9
should be implemented.
