I reviewed a draft of the ip clearance form. Some things to do:
1) A trademark search for DUCC came back with a hit for some software. This
needs to be discussed with Apache trademarks, to see if we need to change the
name.
2) I didn't understand why the item checking that all parts were under good
licenses was marked "N/A"? (I removed the NA, so it now has no date, for
now...)
3) Some other minor edits
