great idea to have a survey. I just did it and have the following comments:
1. The question about the participation to the initiative would be better
addressed to the individuals and not for the entire team as it's hard for one
person to talk for the team.
2. Not sure what a comprehensive training curriculum is.
3. The open questions seemed quite repetitive. Maybe it would be better if you
could merge them with the multiple choice ones. For example, having the
multiple choice and then an "Elaborate" or a more specific but related
questions with a texbox.
